Chavez & Mederos Clinic Book Appointment

Your patients. Our responsibility.

A referral does not end when the appointment is scheduled. We are an independent, locally owned primary care practice, and we treat the members you send us as our responsibility from the first call onward.

Brokers place members with plans. The member's actual experience is decided by something else: whether they can find a provider who takes their plan, speaks their language, and can see them before the coverage year is half over. That last part is where most placements quietly fail — and it comes back to you at renewal.

What we offer the members you send

  • Genuinely bilingual care — Spanish end to end, not an interpreter line. For Spanish-dominant members this is the difference between a plan they use and a card in a drawer.
  • Same-day appointments, generally available for new members, with walk-ins welcome.
  • Care management — CCM, PCM and APCM with proactive outreach, supporting over 1,000 patients.
  • Remote patient monitoring — blood pressure, glucose, weight and pulse oximetry tracked between visits.
  • On-site diagnostics — diagnostic ultrasound and in-house blood draws, so care does not fragment across the valley.
  • In-home visits for members with mobility limitations, usually the hardest need to place.
  • DOT physicals for commercial-driver clients and small trucking accounts.
  • A real person answers the phone. No automated system between you and the clinic.

Conditions we manage

Diabetes, hypertension, cardiovascular disease, COPD, chronic kidney disease, and members carrying several of these at once — the population where continuity actually moves outcomes.
