Primary care for your whole family

Primary care is the part of medicine that works slowly and quietly. It is the annual visit that catches something early, the blood pressure that finally settles after three adjustments, the referral placed by someone who already knows your history.

Most people meet the healthcare system at its worst moments — an urgent care at nine at night, an emergency room, a specialist who has never met them before. Primary care is the opposite of that. It is one clinician who sees you when you are well, so they recognise it when you are not.

At Chavez & Mederos that clinician is one of three bilingual Nurse Practitioners, and you see the same one each visit. For most of our patients that continuity is the whole reason they stay.

What primary care covers here

  • Annual wellness exams and physicals, including school, sports and employment physicals
  • Chronic condition management — type 2 diabetes, high blood pressure, high cholesterol, thyroid disease, asthma, COPD, chronic kidney disease
  • Acute and sick visits, usually the same week and often the same day
  • Lab work drawn in the clinic, plus age-appropriate screening
  • On-site diagnostic ultrasound, so imaging does not mean a second appointment across town
  • Medication management and refills, reviewed properly rather than renewed on autopilot
  • Referrals and coordination to specialists, imaging and hospital care — with follow-up afterwards so results do not disappear
  • Care management between visits for patients living with several conditions at once

Common questions

Are you accepting new patients?

Yes, all three providers are accepting new patients. You can request an appointment online or call the clinic. Most new patients are seen within the same week.

Do I need a referral to be seen?

No. Primary care does not require a referral. If your plan asks you to name a primary care provider, call the member services number on your insurance card and list Chavez & Mederos Clinic.

How long is a first visit?

Plan on about 45 minutes. A first visit is longer than a follow-up because we are building the whole picture rather than solving one problem.

Will I see the same provider every time?

Yes, unless you ask otherwise or your provider is away. Continuity is most of what makes primary care work.

Do you speak Spanish?

Yes — from the front desk through the exam to the follow-up call. All three Nurse Practitioners and our staff are bilingual. This is not an interpreter line.

This is not for emergencies

If you are having chest pain, difficulty breathing, sudden weakness or numbness, trouble speaking, severe bleeding, or any symptom that frightens you, call 911 or go to the nearest emergency department. Do not use this website, and do not wait to see if it passes.
