Primary care for seniors and Medicare patients

Older patients rarely have one problem. They have four conditions, nine medications, three specialists and nobody joining it up. Joining it up is the job.

We accept Medicare and Alignment Medicare, and we can be listed as your primary care provider. For patients who qualify, we also run structured care management between visits — see Chronic Care Management.

What senior care includes

  • Medicare Annual Wellness Visit — the yearly visit Medicare covers, used properly rather than treated as a formality
  • Full medication review, including the ones prescribed by other doctors and the ones you buy yourself
  • Chronic condition management — diabetes, blood pressure, heart disease, COPD, kidney disease
  • Fall risk, balance, vision and hearing — the things that most often end independence
  • Memory and cognition concerns, discussed frankly with the family involved
  • Coordination with specialists and with hospital care after a discharge
  • Advance care planning — see the advance directive form in Patient Resources
  • Home visits when travelling to the clinic is genuinely unsafe or impractical

After a hospital stay

The weeks after a discharge are when people get readmitted. If you or a family member has just come out of hospital, call us — that is exactly the visit worth arranging quickly, and we can often do it at the house.

Common questions

Do you accept Medicare?

Yes. We accept Medicare, and Alignment Medicare among Medicare Advantage plans. Check our insurance page and call to confirm your specific plan before scheduling.

Can I name you as my primary care provider on my Medicare Advantage plan?

In most cases yes. Call the member services number on your card and ask to assign Chavez & Mederos Clinic. Call us first if you would like us to confirm we participate in your plan.

What is a Medicare Annual Wellness Visit?

A yearly visit Medicare covers that reviews your health risks, medications, screenings and care plan. It is not the same as a physical exam, though we frequently combine the two visits sensibly.

Can you see my parent at home?

Sometimes. It depends on where they live, what they need and how their coverage works. Call us and we will tell you straight away whether we can.

This is not for emergencies

If you are having chest pain, difficulty breathing, sudden weakness or numbness, trouble speaking, severe bleeding, or any symptom that frightens you, call 911 or go to the nearest emergency department. Do not use this website, and do not wait to see if it passes.
